Creating a Fund and remembering a loved one

There are 2 ways of creating your Tribute Fund, allowing you to choose the way that suits you the best. 

  1. Online - please fill in the details below, and you will be guided through the simple process. If you already have a Tribute Fund and would like to take it online, please make sure that you put the details in the online Fund exactly as they appear on your Tribute Fund pack.
  2. Over the phone - you don’t have to create a Tribute Fund online, if you would like to set one up offline please call 0800 953 0113.

Whichever way you create your Fund all gifts and donations made in memory of your loved one will be recorded in your Tribute Fund. We'd like to send you a Tribute Fund welcome pack which contains all you need to know about ways to grow your Fund.

Please note that if your Fund was set up ‘offline’ by Guide Dogs, it won’t automatically be set up online too. However, you can easily do this by completing all of the details above, please use the name of your Tribute Fund as it appears in your Welcome Pack.

Naming a puppy in memory

Many supporters use a Tribute Fund to raise the funds needed to name a puppy in memory of their loved one. If you name a guide dog puppy, you'll receive updates and photos as you follow their journey as they aspire to become a future life-changer.

Naming a puppy and remembering a loved one is a wonderful tribute. It ensures that their spirit, and in many cases their name, lives on in a puppy, creating lasting memories.

Tribute Funds are often set up in order to continue supporting your loved one's favourite charity – carrying on their interest and support in our work and creating a lasting tribute. 

Whether it's a person or an animal being remembered through the Fund, your support will help us create more life-changing partnerships. Family and friends can raise funds for, and donate in memory to, the Fund whenever they want, and we can help you commemorate the life of your loved one on special dates and anniversaries.

Please note that any Gift Aid, whilst gratefully received, can’t be counted towards Name a Puppy targets. Not all donations are eligible for Gift Aid to be claimed, and this ensures everyone is raising the same amount.
